Student Employment Forms

Hiring Supervisors: Please submit required forms 1–4 (listed below) for each new student employee at least three days prior to the starting date. Complete hiring packets must be submitted by a full-time staff member or student manager to the Business Office. Incomplete packets will not be accepted.

Student Appraisal Program

The student appraisal program is for all student employees and student leaders (CSO officers). Before beginning the appraisal process, please review the Student Evaluation Policy 

Forms and Rubrics

Student Evaluation Form (must be completed for all students on payroll)

Student-Development-Examples (resource for scoring rubrics)

Learning-Outcomes-Rubric (one form completed per unit/work area)

Student-Employee-Rubric (must be completed for each student employee, IN ADDITION to the evaluation form)

Student Leadership Rubric (students not on payroll, but involved in CSO's)

All forms must be completed for student employees/student leaders by the last class day for each semester. Students will receive automatic pay increase after two successful evaluations ($.25 for meeting expectations, $.40 for exceeding expectations)
